Former CEO of Octiv brings visionary business and entrepreneurial leadership to growing online form solution platform
INDIANAPOLIS (May 12, 2017) — Formstack, a software-as-a-service (SaaS) company that helps businesses manage information and automate workflows through a suite of powerful online form tools, today announced the addition of award-winning business leader, entrepreneur and technologist Dustin Sapp in a new chief operating officer (COO) position.
Sapp joins the Formstack team at a time of sustained growth, with the company anticipating continued revenue expansion in 2017, a 50 percent increase in its remote workforce of already nearly 100 and further product enhancements to scale Formstack’s online form solutions.

In his new role, Sapp will be charged with leading and managing corporate strategy, analyzing market trends and advising the leadership team on the company’s overall growth and market innovation. Specifically, Formstack is expanding its customer base of 15,000 into high-growth industries such as human resources, healthcare and higher education.

Sapp is an award-winning business leader, entrepreneur and technologist who has been named to American Venture Magazine’s Top 40 Under 40, Indianapolis Business Journal’s Forty Under 40 and Indiana Chamber’s Dynamic Leader of the Year. Most recently he served as Chief Executive Officer of Octiv (formerly TinderBox), where he was responsible for leading and managing corporate strategy, growth and product innovation. He co-founded the company, wrote the initial code for its technology and has since led the organization to become a leader in online sales productivity technology. Sapp will continue to be involved with Octiv’s board of directors.
Before Octiv, Sapp served as the president and co-founder of Vontoo, a voice technology platform, and co-founder of NoInk, a mobile sales productivity solution for the medical device industry. He holds a Bachelor of Science in Computer Engineering from Rose-Hulman Institute of Technology.
